Prepare and complete the following documents

Out-patient treatment

  • Claim reimbursement form
  • Doctor’s certificate stating the diagnosis of condition treated – For skin disease treatment, doctor is required to identify specific name of disease as well as prescribed medicine in order to ensure that your claims will be assessed in a timely manner
  • Original receipt with itemized medical expenses
  • Copy of membership card
  • Copy of ID card

In-patient treatment

  • Claim reimbursement form
  • Doctor’s certificate stating the diagnosis of condition treated
  • Attending Physician’s Report completed by your treating doctor
  • Original receipt with itemized medical expenses
  • Copy of membership card
  • Copy of ID card

Please note that Aetna cannot proceed with the claim settlement if we do not receive the original receipt


Submit the completed original forms and documents to Aetna

There are two ways to submit the documents to us

  • Bring in the documents to us in person at any Aetna office
  • Mail the documents to Claim Department, Aetna Health Insurance (Thailand) Public Company Limited 98 Sathorn Square Office Tower, 14th-15th Floor, North Sathorn Road, Silom, Bangrak, Bangkok 10500

Please note that the supporting documents for claim reimbursement must be submitted within 30 days after the treatment (outpatient treatment) or after discharge from the hospital (inpatient treatment) and it is highly recommended that the claims documents be sent to Aetna by registered mail to avoid the loss

You may request the claim to be reimbursed by

  • Electronic Money Transfer

Claims settlement period

For claims with completed documents and no further claims review required, the settlement shall be made within 15 days
